T5表是银行的customer service 打印出来的简单的表（标准格式的找不到了），上面没有box号码。所以来问一下是不是利息要添到T5 slip summary 中的box13(interest from Cdn sources)？这个表最上面的Description要填些啥啊？然后这个T5 slip summary就没啥可填的了吧？

Yes. put the number of interest in box 13 , and in descrption you only need to put the name of bank . that's it.
