•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
* y& [) V- I2 l: {, j•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.
