When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic % m7 t' w, X3 f6 f1 Dcondition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and . r9 X. \3 c4 Q# q, Q. M• there have been no new symptoms or change in symptoms; and/or7 A' W+ j9 j4 l3 |4 u
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.) `* N) x/ i+ D0 R
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
