哪位大侠给解释一下，俺随便找个人就可以做还是必须到专门的机构去做哇，还有这个affidavit有专门的固定格式吗？

不是谁都可以做。记得在INSTRUCTION里专门有一页列出是谁可以做这个。

我以前做过一个类似的。当时找一个机构做的，由它的一名志愿者帮忙翻译，之后在下面有个证人的签字和盖章，是一位COMMISSIONER FOR OATHS的。这样就可以了。
