9 l I4 t. A' A- W2 d2 M# Z8 I* g8 KThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

1 M% f! Q; |1 p+ MThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.8 J9 a' U* K5 S8 b0 I1 r

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

Qualifications/Experience
. u+ S3 X+ Z6 |1 E/ P8 `" Q1 f1 D• Baccalaureate degree in Nursing preferred; T4 n4 T) |+ \3 D- \0 g/ @+ d
• Preference given to those with at least 5 years nursing experience in primary care setting.
0 ]" s# Q5 V; E" u/ S• Current CARNA Registration- x9 u+ r9 M/ l$ U0 q
• Current CPR – Healthcare Provider
4 {7 ]# h% H$ r8 U• Must have own vehicle and valid driver’s license
