POSITION SUMMARY

The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

, [; c* O( a0 }  x& u/ j) {; KThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.( U% o+ d4 b/ r7 d' B; T0 ]0 W
•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.
  s$ G5 }& ^* f; a9 U; o5 ]' F•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

Clinical Role

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  6 p, m  M+ }# N5 ?) k3 W! I2 S
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services.
" }5 ^4 K* S8 x& O- r) t•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.- E: p& I4 R# t1 l7 F. a9 d
•        Ensure the maintenance of accurate records in line with practice policy.( ^) h7 y/ N. C0 s5 }+ Y
•        To assist in developing and implementing patient self-management programs.

•        To offer training support for other team members and assist in their professional development.0 s# C: Z' V4 l. r

•        He/she should be able to work in a team while respecting the views/opinions of others.
5 B% B7 `: b4 @& s•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.! n) `& A  e$ L8 ~% W8 O
•        Requires the ability to work independently and prioritize workload.- L7 Q$ Z! e9 O, \2 U2 t6 Q
•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.3 `/ h& ?- [) ]$ v+ {
•        To participate in the Network’s education program.

SELECTION REQUIREMENTS:) y  l* b; w+ K$ z* E, @: ?

/ K5 ^# i9 z; G0 Z$ ^Qualifications/Experience
  |  y6 V# o0 k* Q& g$ C) P•        Baccalaureate degree in Nursing preferred/ c% I1 i( q- ?# ~+ \
•        Preference given to those with at least 5 years nursing experience in primary care setting.  k1 F3 z* @) t8 W2 U
•        Current CARNA Registration: c+ B6 _1 N, v# K- D
•        Current CPR – Healthcare Provider
1 q; j5 f$ A& \# N9 Q•        Must have own vehicle and valid driver’s license
5 A2 |0 W$ o7 x, y•        Knowledge of, and ability to use, electronic medical records& v+ p4 y  v# m! P' x2 ~8 t1 k2 P- X
