1. sub floor did not hold tight to the floor joists. Screws are doing better than nails for this purpose.$ M0 s& Z( q( M1 ~
2. supporting beams are a bit too far away from each other.

For 2, you may need someone with engineering knowledge to find out, but there should be a building code governing the span distance. For 3, you can go down the basement to do a visual check yourself.: R5 O7 D M9 w5 V& w9 t
