provide supplemental support to newcomers who have complex needs.

" w9 R$ s" L, G0 V, X$ B1 W Nature of Work: The Social Worker will report to the manager of mental health services. ! q b# |9 d* L* Y9 P( B
The position builds working relationships with New Canadian Health Network partners’ staff,
$ X7 U7 L; H- i% qnewcomers accessing these services and other institutions and service providers. The Social
3 x ] N7 Y/ P. t+ q: ?Worker will assist in case planning, conferencing and providing support and advocacy as
' t3 v; X& ?+ l% M5 r+ Uneeded. As required, the Social Worker will assist in accessing community supports for 3 q( {& P* u/ q3 J5 b& Z' x
newcomers and their families. The Social Worker provides holistic and flexible initial
; a' I% a& O5 f# b# S+ massessment of life circumstances, needs, goals and aspirations and adheres to the client-
% a5 R/ d; m4 U$ r) W" [centered delivery model.

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support 6 t4 Q# J! l6 V0 c

Work in partnership with settlement workers or cultural brokers to provide supplemental
' K1 P) ^7 i7 z. jcase plan support for clients who have complex needs.

experience in Social Work or related human service fields. - n9 i$ A5 [* K
Extensive knowledge of community resources, including in depth understanding of family ) P5 W& H/ ~ [; A; e; i2 C. s

Experience/facility with MS Office computer applications
* S, s, D2 w1 ]* F7 S8 w Proficiency in both official languages is an asset, as is fluency in other languages

Experience: Minimum of 3 years experience working in a related field
