Primary purpose: The Social Worker works in partnership with settlement workers or

provide supplemental support to newcomers who have complex needs. ' v# f% u, }4 W, i& J

Nature of Work: The Social Worker will report to the manager of mental health services.
2 G% m- c6 n( M+ I+ iThe position builds working relationships with New Canadian Health Network partners’ staff, 9 g% N. h7 S+ {! x3 x4 t
newcomers accessing these services and other institutions and service providers. The Social
9 _0 l( D, v8 b7 |! v) M4 q7 XWorker will assist in case planning, conferencing and providing support and advocacy as ; f0 i t7 Y, i3 I7 W
needed. As required, the Social Worker will assist in accessing community supports for 8 A J7 `+ T+ X' g0 S
newcomers and their families. The Social Worker provides holistic and flexible initial
9 ^4 M- ?& x, N) F# passessment of life circumstances, needs, goals and aspirations and adheres to the client-
" N* G T' p" k$ rcentered delivery model.

Provide intake, needs assessment, referral, case plans and action plans to support ! o# p' d, @# f" T5 b5 a! b
newcomers in settling and fully participating in community.

case plan support for clients who have complex needs.

Extensive knowledge of community resources, including in depth understanding of family

Experience/facility with MS Office computer applications 7 G- I: U A& J3 X7 R
Proficiency in both official languages is an asset, as is fluency in other languages
