Primary purpose: The Social Worker works in partnership with settlement workers or " o0 {* R" U+ c8 y: |& g
community cultural brokers from New Canadian Health Network partner organizations to 1 U8 e$ Q& |" |, A! B8 E/ J
provide supplemental support to newcomers who have complex needs. $ p7 }3 Q+ ?- I; V
6 a, b9 t/ Z; ~: d6 n5 Y% M Nature of Work: The Social Worker will report to the manager of mental health services. ^( r0 T5 \* l z; p* m
The position builds working relationships with New Canadian Health Network partners’ staff, 9 @* B4 C. \- p3 X( {. l) H
newcomers accessing these services and other institutions and service providers. The Social % U& q' f7 ? z4 ^+ F* G6 t \
Worker will assist in case planning, conferencing and providing support and advocacy as
0 u3 r0 N' o/ z: i! G/ |( kneeded. As required, the Social Worker will assist in accessing community supports for 1 n2 |0 n! w% l6 l. ^
newcomers and their families. The Social Worker provides holistic and flexible initial t; f" e# Q$ f9 b% K
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support
# o* g9 L8 R9 \" tnewcomers in settling and fully participating in community. & g; C! Q4 Q4 A W
Work in partnership with settlement workers or cultural brokers to provide supplemental
# [& i6 U4 h( C4 h8 M0 L: d% K) ^case plan support for clients who have complex needs.

Qualification
4 j: M M p$ Z% g; c ; b6 ~! H( K. T( _8 N! p( K
Relevant undergraduate degree or diploma, or equivalent combination of education and 3 m7 }2 G6 `4 ` a
experience in Social Work or related human service fields.
' @; _1 s7 g4 t/ y; o. {) Z Extensive knowledge of community resources, including in depth understanding of family
0 e1 w9 U2 e( E/ @: x' Ssupport needs
3 t' H4 M C) a% x Experience/facility with MS Office computer applications / G4 R3 u0 ^6 v% z0 G$ h
Proficiency in both official languages is an asset, as is fluency in other languages
0 \8 V. F' M% S5 o Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate * Y- M, x( p7 F
insurance coverage for work 3 D$ u+ u* q* I) i) o7 Q v( w
( r5 A% O+ y( [: |Experience: Minimum of 3 years experience working in a related field % `4 d; t& w% U& d, t$ b
