Primary purpose: The Social Worker works in partnership with settlement workers or 7 [5 Z3 T$ {+ T5 Q8 g1 k8 Y9 d
community cultural brokers from New Canadian Health Network partner organizations to
4 A8 {$ ]6 y: V+ Aprovide supplemental support to newcomers who have complex needs.

$ x) W" K- n# T: Q# K" ` Nature of Work: The Social Worker will report to the manager of mental health services.
; b+ R6 v/ g+ S- Q% ]The position builds working relationships with New Canadian Health Network partners’ staff, & s- z( W Y5 A7 I P( m* A
newcomers accessing these services and other institutions and service providers. The Social 4 O( X/ ^. F, s
Worker will assist in case planning, conferencing and providing support and advocacy as ( ^" f& n9 \# N& r7 m2 y
needed. As required, the Social Worker will assist in accessing community supports for
; u z/ q% F: J" v' K* [newcomers and their families. The Social Worker provides holistic and flexible initial
& i0 _- \7 i' ^* ~8 e* n l5 |assessment of life circumstances, needs, goals and aspirations and adheres to the client-
- G" _$ T, E( X$ V. \3 p4 y& Z) Ocentered delivery model.

Work in partnership with settlement workers or cultural brokers to provide supplemental 6 l" E9 m" s a/ {6 s
case plan support for clients who have complex needs.

Qualification
" }/ [6 U3 Y2 G/ O, ]/ `: I V1 w$ d9 [ K/ h8 o/ Y: H! M ~
Relevant undergraduate degree or diploma, or equivalent combination of education and
5 y& @' ]5 i# I) d, Nexperience in Social Work or related human service fields. 4 q( u, X7 T1 I% H* p, g! u9 A
Extensive knowledge of community resources, including in depth understanding of family 1 \; g& g7 v5 @4 ?0 ^' x* |
support needs : p* X% A. J; e# o
Experience/facility with MS Office computer applications 5 Q- T& B# F8 ?; w% v* p
Proficiency in both official languages is an asset, as is fluency in other languages $ u) Q& i1 |6 W& |4 ~
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
