Primary purpose: The Social Worker works in partnership with settlement workers or ) s$ O4 X" Y3 }$ S* z) c# ^& _
community cultural brokers from New Canadian Health Network partner organizations to . y2 f" O. Z3 N4 l# ?2 y% U6 p; V
provide supplemental support to newcomers who have complex needs. ' z; Y( I6 X; h# h* [5 `

Nature of Work: The Social Worker will report to the manager of mental health services. - |0 j; P! ?: E9 [3 X% l! g0 `3 F- ?
The position builds working relationships with New Canadian Health Network partners’ staff, ( E! D9 Q: Y# |" W' s5 Q: W9 `
newcomers accessing these services and other institutions and service providers. The Social 4 m1 ?* k3 B8 C, T) W
Worker will assist in case planning, conferencing and providing support and advocacy as
& O* b8 E0 ~1 D0 eneeded. As required, the Social Worker will assist in accessing community supports for
; O6 i+ U1 S# anewcomers and their families. The Social Worker provides holistic and flexible initial : F! ~/ [& A1 W
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support - G' c* E8 Q4 x+ |2 @" l! l
newcomers in settling and fully participating in community.

case plan support for clients who have complex needs.

Qualification
. N& C1 M$ @) {* u) Q * Q! f' y8 t, C& j
Relevant undergraduate degree or diploma, or equivalent combination of education and
7 |3 X9 b9 U6 F) o3 Hexperience in Social Work or related human service fields. : T% O* U% [# E
Extensive knowledge of community resources, including in depth understanding of family ! d& J; l1 l5 y) L2 J5 E
support needs $ }8 d0 ^. D/ R
Experience/facility with MS Office computer applications
5 U' u) u0 P7 D4 h u Proficiency in both official languages is an asset, as is fluency in other languages - E% K2 X3 T( v( p4 O0 M
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate % T, \( Z, M) y6 H
insurance coverage for work

Experience: Minimum of 3 years experience working in a related field & |2 p5 o6 A7 F0 d; n
