4 H' n L' j" eAbstract: position offers both crisis/on-going support for individuals/couples with chronic or complex mental health issues + those who face multiple barriers to independent living to enhance their ability to live successfully within their community by reducing barriers involved in meeting their economic, social and personal needs. - M0 Z+ T) H. d7 I& Q4 o2 ]5 o

Position is also responsible for providing crisis intervention, intensive outreach support, advocacy, information and referrals to internal and external resources as appropriate. , g/ r/ X+ }. b
9 c* ]( x4 n, j# y5 Z7 r' XEducation/Experience: Bachelor Degree or Diploma in Social Work, Rehabilitation Services or equivalent + relevant experience; familiarity with issues of poverty/homelessness; AB Class 5 license, satisfactory drivers abstract and own vehicle.' S# `) X3 @, D2 j( e4 G) j

Skills & Abilities: Strong written/verbal communication skills; effective assessment skills and ability to work from a strength-based perspective; ability to develop goal plans with the program participant; ability to establish rapport and trust with the program participants; strong problem solving skills and an ability to flexibly manage multiple tasks.+ H: d8 V6 s4 S9 b( R3 D& W X
) ]% T$ `1 E. m& [; d" TPLUS: Knowledgeable about community resources; ability to be self-directed and work well within a team; ability to develop productive working relationships with program partners & other community agencies; ability to develop service plans with program participants and provide follow-up to support them in the achievement of their goals.% ?5 y# K( u1 f2 W$ U1 J. K& _
