Abstract: position offers both crisis/on-going support for individuals/couples with chronic or complex mental health issues + those who face multiple barriers to independent living to enhance their ability to live successfully within their community by reducing barriers involved in meeting their economic, social and personal needs.

( Z" F" b' J/ t2 Q# KPosition is also responsible for providing crisis intervention, intensive outreach support, advocacy, information and referrals to internal and external resources as appropriate. 5 B$ E/ h N) K0 x; a
0 F6 I& Q% {& [6 q% T; UEducation/Experience: Bachelor Degree or Diploma in Social Work, Rehabilitation Services or equivalent + relevant experience; familiarity with issues of poverty/homelessness; AB Class 5 license, satisfactory drivers abstract and own vehicle.! B+ |4 j8 t: I1 H( A

Skills & Abilities: Strong written/verbal communication skills; effective assessment skills and ability to work from a strength-based perspective; ability to develop goal plans with the program participant; ability to establish rapport and trust with the program participants; strong problem solving skills and an ability to flexibly manage multiple tasks.

PLUS: Knowledgeable about community resources; ability to be self-directed and work well within a team; ability to develop productive working relationships with program partners & other community agencies; ability to develop service plans with program participants and provide follow-up to support them in the achievement of their goals.- ~. s2 \) w. u2 c* o2 t% _6 z% I
