Target:        Low income families with children/youth aged 3-17 residing in Edmonton
6 r: v& [" `6 Y7 G' [2 q; ?5 F! oDate        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.

' c7 y9 |! r6 V+ k8 f0 q; C1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2 H* R. _. C5 o# y4 ^$ G+ i5 P2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
# {6 {3 ]1 E3 c/ k& v5. Children are not required to be present at the Registration Day

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
