  R( m! \; d7 A4 v. s, k4 [Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
/ k4 H  F* P9 h# W8 b) [- _/ t9 J3 XPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
Remarks:
5 I" M* O1 _# `: q' o5 H1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
5 @3 K* X. o! Q! ^( Q2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
0 d6 H/ B- b4 T2 U" }4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
6 b; t6 M6 k' \6 x2 N3 W8 {# m6. Registrations will be accepted on a first-come, first-served basis

: H1 t4 p) D6 [We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
