Low Cost Summer Programs Registration Day

Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
  e+ G1 q; g6 T$ |8 A) XPlace        ASSIST Community Services Centre (9653-105A Ave)
& [2 H. r$ Z" `( ?Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
0 T: V) a7 k, A: v2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
9 m3 D2 g: b# K* B/ |' O8 R3. Each child is limited to 1 to 3 programs per organization
, T! b* J; K8 x4. Information on Child’s swimming level when registering for swimming classes
8 J- e1 O2 u5 ]  B0 N+ E5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

5 C. T8 _; d/ a% h+ a+ T% _We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
