1.Print the Leisure Access Program Application Form.The form is also available:

Monday-Friday except holidays, 8:30am-4:30pm.
$ k8 |1 ]  }$ g2 _•By calling 780-496-4918 to have one mailed to you.
. x0 q! r9 q. s$ a•For individuals under custodial care, please also submit the address verification form.
