1.Print the Leisure Access Program Application Form.The form is also available:

' }% h& f0 {1 [3 d Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
, I( L$ ^% O* r•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.
