•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
+ s+ C5 R5 @4 f9 o2 l) ` Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
* i( k9 @3 {! A% c+ [9 L  u) A4 ~+ N•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.
