M) V! ]0 T# m0 D; gThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.& |3 m6 m8 {# w

/ }& R* |$ q% K: K6 E& {The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.. k( `( q6 \1 X0 e4 z

•        To work collaboratively within a family practice environment to enhance the delivery of primary care services.! |: W* [" H8 G- z2 {
•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.3 N2 }: K. ^# y# e
•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
0 }# r, ~" O- F- z# h) S•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.  A8 y, A  V( a2 G% R
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population." P, Z* l1 w" ?
•        Educator, communicator, advocate, coordination/navigation, supporter

7 \, _! d$ p; Z( Z/ e•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.    d+ \4 P% N- L1 a3 u
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services. / N4 Z4 b+ g6 p
•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

•        To assist in developing and implementing patient self-management programs.# Z0 h7 E8 q& J1 \
•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.2 y( g, g3 ^+ z
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.

•        The primary care nurse should have excellent written and verbal communication skills.8 j+ T, k. E( V% w+ Z' \. Z
•        He/she should be able to work in a team while respecting the views/opinions of others.
9 u# B. ^0 n9 l) ^( g3 S•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.- Z) X* a4 U1 H8 m* a
•        Requires the ability to work independently and prioritize workload.  F# h/ m$ w# N4 Q  g
•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan." H0 b0 d2 |- {& i0 k" ~
•        To participate in the Network’s education program." O; ?: h* a3 H+ h5 b

SELECTION REQUIREMENTS:

4 O' i! a! `# h$ |, OQualifications/Experience
5 T2 P2 b! c! l8 G•        Baccalaureate degree in Nursing preferred8 \* Q. T$ V" W3 u$ \( V4 t+ V4 u
•        Preference given to those with at least 5 years nursing experience in primary care setting.% C! ~9 B& z$ \! x; \
•        Current CARNA Registration. C; W$ u, ?8 v8 Q; H" X
•        Current CPR – Healthcare Provider& o7 V9 [9 G; }7 u/ k
•        Must have own vehicle and valid driver’s license
) Y* t/ h1 i. l; X•        Knowledge of, and ability to use, electronic medical records
