4 d0 \+ L; m5 s+ _ g( FThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population. `4 g5 l9 u+ Y$ A c

The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.1 l) p9 w/ I& f9 C9 W) S4 u

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

• To assist in developing and implementing patient self-management programs.

• The primary care nurse should have excellent written and verbal communication skills.

Qualifications/Experience3 @; J- P! ?3 t
• Baccalaureate degree in Nursing preferred
4 A+ c! @0 Y+ _7 g' R) O• Preference given to those with at least 5 years nursing experience in primary care setting.3 ]7 D$ b+ z7 [( L( x0 [7 d
• Current CARNA Registration( @+ J+ C0 H" ]0 y1 F
• Current CPR – Healthcare Provider
8 l6 x+ `1 c& r9 Q% [, P# E4 k• Must have own vehicle and valid driver’s license
