POSITION SUMMARY

The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population." d' C( o: s0 A

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.$ |- H8 y0 q% c/ |% a( J( |8 `# r9 l
• To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.0 N: c" m% T( L2 T
• Ensure the maintenance of accurate records in line with practice policy.+ H1 m. O$ L4 k4 B! e2 z
• To assist in developing and implementing patient self-management programs.# Q+ m# n7 r& \; ]7 _. V6 K4 O
• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.# P6 G7 U/ Q* ~1 l+ \
• To act as a resource to other multidisciplinary team members, physicians, and community resources.

Qualifications/Experience7 s: a/ U5 L! J3 c7 D7 _# K; N
• Baccalaureate degree in Nursing preferred5 {# |% c' Q4 x+ W2 }( ?
• Preference given to those with at least 5 years nursing experience in primary care setting.
( m) A! s0 J8 E& m/ y• Current CARNA Registration
8 {. M' P n; P* y• Current CPR – Healthcare Provider
# Y) O0 ^3 r: D* n* M- F# z1 y• Must have own vehicle and valid driver’s license. ]* l, h+ c6 {8 ^/ D' f% n& v
• Knowledge of, and ability to use, electronic medical records/ c! R5 l$ o" u. ~1 ~1 e) `' d
