name，email，telephone number，是否UA student或staff， alumni（不是学生需要交membership fee，好像是$12）， onecard number？

; r5 h4 Z1 Q/ V0 O% ]. m* S男子三对三排球，报名时间9月10日，需至少3名male player，报名费$20每队。
比赛时间9月20-10月20日，每周一和周三晚上，共四周。

Levels:
$ [* Q' p% R7 J8 z# ^  n6 kRecreational:  These teams are composed of players with no prior competitive volleyball experience.
2 ]/ H/ k* U3 D( [Semi-Competitive:  These teams will have a mixture of skill levels on their team.  It is intended for teams composed of  a mixture of skill levels or with players who play pick-up on a regular basis or who may have played in high school.
) K" ^. r( W7 h+ l5 y0 o* i2 c2 lCompetitive:  For teams with players who played on their high school squad or who still currently play on a team (ie. City league, etc.)

男女混合排球，报名时间10月5日，需至少男女各3人，报名费$70每队。
: B- N9 e  c# `4 w比赛时间10月20-12月8日，每周一至四某个晚上，报名时需要选周几固定下来。
