Primary purpose: The Social Worker works in partnership with settlement workers or
* f* y& P( a- @7 q7 L" I! w4 \1 Pcommunity cultural brokers from New Canadian Health Network partner organizations to $ D0 g4 V1 j1 F5 Z' u9 Q
provide supplemental support to newcomers who have complex needs. 7 k' J- e6 V) w$ F5 n' X# z, _
0 I# P# `: P, w6 T+ \" y Nature of Work: The Social Worker will report to the manager of mental health services. # v: |# u3 \ c" y! R$ P. a
The position builds working relationships with New Canadian Health Network partners’ staff,
: Z' q- P! J! |newcomers accessing these services and other institutions and service providers. The Social
: c0 H$ O, r, Q9 X Y% c* d$ QWorker will assist in case planning, conferencing and providing support and advocacy as + K# c- p3 d& |& F: U2 X) e
needed. As required, the Social Worker will assist in accessing community supports for
4 G3 \# m; {- L6 A9 {newcomers and their families. The Social Worker provides holistic and flexible initial
, D. l, j: y8 y2 V1 ^/ s) |$ |assessment of life circumstances, needs, goals and aspirations and adheres to the client-+ s' y" s$ q( b3 R0 n; H
centered delivery model.

Accesses comprehensive, up-to-date information about services and resources available

Providing holistic information or advocacy services for immigrants and refugees served. 3 X/ I) y. p, O6 D( i5 p
Developing of social networks and community supports for newcomer families.
/ O& H% Y" n, R- S Provide intake, needs assessment, referral, case plans and action plans to support
4 q2 T9 d; F% M9 o" J: [newcomers in settling and fully participating in community.

Advocate on newcomers’ behalf to programs, institutions and service providers.

Relevant undergraduate degree or diploma, or equivalent combination of education and
, T1 y5 C/ y/ E/ {/ ~9 [9 Z+ rexperience in Social Work or related human service fields. ( ]3 x2 g1 w; O% y# N5 s* \
Extensive knowledge of community resources, including in depth understanding of family _8 i1 B$ c' c. M
support needs
' n- R) Y& i7 g" \. U" ^. J! `; K Experience/facility with MS Office computer applications
|9 F. N* N, G5 W% P& K9 n: R Proficiency in both official languages is an asset, as is fluency in other languages
3 J( E9 z) @1 ^6 d) R- P1 h Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 5 `' r/ x! f7 `8 z# f# ]) B' w/ S

Experience: Minimum of 3 years experience working in a related field # Y0 S* i+ j6 I
