community cultural brokers from New Canadian Health Network partner organizations to
" R+ O* F9 f1 e6 f) J3 `& lprovide supplemental support to newcomers who have complex needs.

/ V% f j* y$ l( ? S Nature of Work: The Social Worker will report to the manager of mental health services. 3 k4 h9 ^% V. t8 w2 w& G6 i* W
The position builds working relationships with New Canadian Health Network partners’ staff, 4 r4 O$ D( f6 g
newcomers accessing these services and other institutions and service providers. The Social
/ n6 a$ O' C/ v( ~- `4 ZWorker will assist in case planning, conferencing and providing support and advocacy as / Q$ x5 s& U h) B8 E- c
needed. As required, the Social Worker will assist in accessing community supports for 5 P' y7 H6 o9 v7 h8 g9 T; C8 A9 n
newcomers and their families. The Social Worker provides holistic and flexible initial
0 ~' n0 {5 Q7 g$ ?7 ^% V) Yassessment of life circumstances, needs, goals and aspirations and adheres to the client-

Developing of social networks and community supports for newcomer families.

case plan support for clients who have complex needs.

Qualification # W) r2 Q+ h/ t
* {4 X5 K+ }# ]; r Relevant undergraduate degree or diploma, or equivalent combination of education and
4 \0 f( O; k9 B0 v% ?5 {3 V/ y2 Sexperience in Social Work or related human service fields. 8 @, X, r" [+ Q+ o \% ~
Extensive knowledge of community resources, including in depth understanding of family
5 l/ `. C* X2 L) f+ U0 ]support needs 3 f0 E+ }3 z- C
Experience/facility with MS Office computer applications 9 y) u; p/ J" d1 l2 N& z- C6 B3 p
Proficiency in both official languages is an asset, as is fluency in other languages $ e4 i( w# { ] w
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 9 c( A% G, [" H* W. _8 C2 d( [4 U# F
