Primary purpose: The Social Worker works in partnership with settlement workers or

Nature of Work: The Social Worker will report to the manager of mental health services.
: N+ {) e7 `8 S# L6 |7 YThe position builds working relationships with New Canadian Health Network partners’ staff,
, c& L( s! A$ W6 i; n$ z3 Jnewcomers accessing these services and other institutions and service providers. The Social
) r1 C; \7 H: b7 `8 z) cWorker will assist in case planning, conferencing and providing support and advocacy as
9 V! h# t0 l* o; E' Sneeded. As required, the Social Worker will assist in accessing community supports for 3 m) J3 p0 B+ k5 I$ W/ N6 @
newcomers and their families. The Social Worker provides holistic and flexible initial
5 ?1 d7 t! d6 k+ p1 B4 j7 Dassessment of life circumstances, needs, goals and aspirations and adheres to the client-4 {( m1 i3 F4 ?. }

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental
! C, j* ]1 `3 e% o; _3 Vcase plan support for clients who have complex needs.

Qualification
O" _( d7 {0 _ / J( O% m, H% j% R2 Y
Relevant undergraduate degree or diploma, or equivalent combination of education and ; `$ x& L5 E% \ x/ R) v
experience in Social Work or related human service fields.
- t- G2 O& ?7 I; w3 K! Y1 ^8 M Extensive knowledge of community resources, including in depth understanding of family & p! O1 X' E) \+ v1 X0 w$ j6 \
support needs

Proficiency in both official languages is an asset, as is fluency in other languages # i9 H' s& U: X
