Primary purpose: The Social Worker works in partnership with settlement workers or
, [( v. v) b, C- C% c9 Wcommunity cultural brokers from New Canadian Health Network partner organizations to # [+ h2 Q4 s$ \ k. K+ X" _
provide supplemental support to newcomers who have complex needs.

H# N3 M7 E& [4 Y qThe position builds working relationships with New Canadian Health Network partners’ staff, 2 A) O3 {9 N$ t* V2 i
newcomers accessing these services and other institutions and service providers. The Social 5 ]5 D8 @. L4 [# X ^) M, K
Worker will assist in case planning, conferencing and providing support and advocacy as
7 a& b; g- B* g/ v* m! ^needed. As required, the Social Worker will assist in accessing community supports for
) W* _9 n" o8 n- Snewcomers and their families. The Social Worker provides holistic and flexible initial
! t' j' A3 |- C. E9 s& W# j# ?3 x6 E5 massessment of life circumstances, needs, goals and aspirations and adheres to the client-0 `$ g1 k3 S% ~0 C0 U

Developing of social networks and community supports for newcomer families.

Work in partnership with settlement workers or cultural brokers to provide supplemental 7 E) ?4 N, f9 ~$ ~# h
case plan support for clients who have complex needs.

Relevant undergraduate degree or diploma, or equivalent combination of education and
