Low Cost Summer Programs Registration Day

Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)

" @/ k" r6 r) L$ S8 z5 {) P( mPlace        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.

Remarks:
" T7 k. O$ y8 e  a7 K- Y7 s1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
# e; U! ^, n9 W- g2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
$ i" }' h  ^- R3. Each child is limited to 1 to 3 programs per organization
( I# c5 O, N( E8 Y4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
6 ~$ {5 Q' F* L9 P6 Y7 l% g. M" E6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
