•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
! N8 e% [5 e7 c Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
" S5 p* g0 |! m& r- k! @•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.
