1.Print the Leisure Access Program Application Form.The form is also available:

* z' i! a$ C0 K- z+ e1 Y•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
' W- ?9 i/ D9 o* b6 m- M3 m•By calling 780-496-4918 to have one mailed to you.
+ E$ l4 Y6 v0 n# u2 H' w7 z8 {•For individuals under custodial care, please also submit the address verification form.
