1 r6 {' t9 @$ U4 z•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
: h! b$ l" s/ u' u* C•By calling 780-496-4918 to have one mailed to you.
•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.
