condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and5 S$ d2 ^/ \; W5 c
• there have been no new symptoms or change in symptoms; and/or3 C L) u0 P g9 M% r# n5 m% S
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy. ) R. t: S2 |' I( Y* T# UIf “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
