When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic 1 ]3 X! _- Z( X. `' Dcondition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and7 F" }. I6 T( f- d( t0 h! t" S- }6 j
• there have been no new symptoms or change in symptoms; and/or 4 g! r; T* d$ p6 F• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.9 p7 q5 c ?; g" p5 n/ J
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
