在里面有几个表格
1.Application for a Permanent Resident Card (including the Document Checklist) [IMM 5444]  (PDF, 990 KB) June 2013
2.Supplementary Identification [IMM 5455]  (PDF, 560 KB) January 2012
3.Photograph Specifications for Permanent Resident Cards (PDF, 989 KB)
4.Use of a Representative [IMM 5476]  (PDF, 55 KB) April 2013
5.Instruction Guide [IMM 5445]
另外，对换发的PR卡还有照片的要求，要到有资格的照相馆照相。照相的要求在Photograph Specifications for Permanent resident Cards (PDF, 1.05 MB).
4 s6 l0 \# V* ~/ ^/ u在London Drugs或Sears 的照相馆，都是有资格照PR卡相片的。Sears照相馆比较专业，但价格贵。
