POSITION SUMMARY

2 y$ q2 Z5 K- h+ C0 ?/ TThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.( }) Q0 j7 {( g

The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

• To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.

• To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.

• Educator, communicator, advocate, coordination/navigation, supporter

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

• To assist in developing and implementing patient self-management programs.
; x& ~. Q5 W) ^* b• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

• Requires the ability to work independently and prioritize workload.

$ |, B$ w; n$ {$ o/ o/ k% Y9 M3 X+ x• Baccalaureate degree in Nursing preferred
+ e3 n; z! W# ^& o( o• Preference given to those with at least 5 years nursing experience in primary care setting.( o6 `' E/ D1 \; P6 T# @* [/ H
• Current CARNA Registration
3 }3 e' @1 _$ z- G5 C# e• Current CPR – Healthcare Provider: Q9 q4 I) E" Z4 v4 W% p7 _
• Must have own vehicle and valid driver’s license. A/ N3 O, i& H+ a& I7 d, t/ f" @! }
• Knowledge of, and ability to use, electronic medical records
