The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

• To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines." Y; ?5 [. v6 I+ F& j- z
• Ensure the maintenance of accurate records in line with practice policy.

• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.$ A9 Q k4 U. L
• To act as a resource to other multidisciplinary team members, physicians, and community resources.

SELECTION REQUIREMENTS:

Qualifications/Experience
' G, C1 T( C/ c8 M8 |8 L; i! m• Baccalaureate degree in Nursing preferred
. U" c) X% O& v: g/ S5 q i% h! V• Preference given to those with at least 5 years nursing experience in primary care setting.
: x/ i1 z2 S! R8 M9 O• Current CARNA Registration/ _9 X2 b4 w1 r& q
• Current CPR – Healthcare Provider
% n7 d: B$ n6 `6 `( y$ T7 X2 y• Must have own vehicle and valid driver’s license
( {* H, S: W- N. y• Knowledge of, and ability to use, electronic medical records
