Primary purpose: The Social Worker works in partnership with settlement workers or 8 q. }4 W1 m: S0 w: W( `

provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
% _ h1 y H: ?; g# d8 W# NThe position builds working relationships with New Canadian Health Network partners’ staff, + c4 \( o! _1 h! [
newcomers accessing these services and other institutions and service providers. The Social
7 k0 W3 [6 ~0 T( ~ \; z4 k. wWorker will assist in case planning, conferencing and providing support and advocacy as 1 n% I9 L% V! O% x) d% @* I
needed. As required, the Social Worker will assist in accessing community supports for
E' m* p5 @7 M0 Enewcomers and their families. The Social Worker provides holistic and flexible initial
$ }% Z/ }- Z! A0 H1 J: ~assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support / t0 M [ I' o' u3 x% X

Work in partnership with settlement workers or cultural brokers to provide supplemental 9 {4 g3 X9 z- v. F# j* ^
case plan support for clients who have complex needs.

- j' b; f& r, i, n$ n- C4 A7 y" x Relevant undergraduate degree or diploma, or equivalent combination of education and

Extensive knowledge of community resources, including in depth understanding of family 1 U# I, V5 n" D( j
support needs

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
