Primary purpose: The Social Worker works in partnership with settlement workers or

provide supplemental support to newcomers who have complex needs. ) f. b6 D* Y3 ] ~, H

Nature of Work: The Social Worker will report to the manager of mental health services.
$ k+ i' ?7 w Q V+ v8 y; v" S: tThe position builds working relationships with New Canadian Health Network partners’ staff,
5 @! ~ R. \ R) p+ ?. c. {newcomers accessing these services and other institutions and service providers. The Social 5 o5 W3 J; _+ E# A B
Worker will assist in case planning, conferencing and providing support and advocacy as
8 y4 L. [4 U2 m4 n4 _needed. As required, the Social Worker will assist in accessing community supports for 8 q) U: t; V1 {& C% \
newcomers and their families. The Social Worker provides holistic and flexible initial
" N; Y) K4 r) D' X: ^8 e3 nassessment of life circumstances, needs, goals and aspirations and adheres to the client-) y9 B" R3 i3 @* x' ?/ h
centered delivery model.

Provide intake, needs assessment, referral, case plans and action plans to support 4 j: G) m E8 a0 p/ O8 l) E

Work in partnership with settlement workers or cultural brokers to provide supplemental

Advocate on newcomers’ behalf to programs, institutions and service providers.

Extensive knowledge of community resources, including in depth understanding of family

Experience/facility with MS Office computer applications , s- v' a) T- m7 c5 u3 S9 d; L# {

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
