Low Cost Summer Programs Registration Day

& x; ?; u2 y. n6 h4 E! H8 ~Date        May 14, 2005 (Saturday)
9 G( P1 G. n( s3 k( e8 w0 M- }+ gTime        9:30am – 11:30am
/ A( |4 c3 b) C; p" \# J) SPlace        ASSIST Community Services Centre (9653-105A Ave)
7 T& R" f5 j( }2 vPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
! E1 L$ }# U8 X1 D( N) h! oFees        Ranges from $10 to $35 per week
! p! V4 r, A# G( y5 c) s       
$ l6 T3 m3 T2 t3 N. W! bRemarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
# O4 F) m$ N9 z' D& h4 Y" V4 [- N; H2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
; O  d; y1 Z8 o; `3. Each child is limited to 1 to 3 programs per organization
+ d- @2 P% e% G4. Information on Child’s swimming level when registering for swimming classes
/ w$ R8 t! D& F: b0 s5. Children are not required to be present at the Registration Day

Fee Reduction Pass Applications
) a- j% E. k) ?
We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
