Time        9:30am – 11:30am
1 L4 q- g' D' z/ t2 T# N/ _Place        ASSIST Community Services Centre (9653-105A Ave)
. a1 |- S2 \! r% p/ IPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
& s' _9 a/ o- e3 aFees        Ranges from $10 to $35 per week
       
Remarks:
( X0 ]/ F9 }5 }; j9 ?  t3 @1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
7 h* \$ O/ K3 R+ ^3. Each child is limited to 1 to 3 programs per organization
1 y$ l/ L5 W' N$ |# r4. Information on Child’s swimming level when registering for swimming classes
; K2 O2 x$ }! j0 D5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
