•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
- m  [" w8 \. v Monday-Friday except holidays, 8:30am-4:30pm.
% J/ t( ^% U& B, Z( b0 s•By calling 780-496-4918 to have one mailed to you.
- d8 E' Z, h! m( t•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.
