We encourage you to make every effort to resolve your concern directly with the dentist involved before making a complaint to the Alberta Dental Association and College. Do not delay treatment because you are waiting for the outcome of a complaint.

' f5 D* K( X6 k) K \8 OIf you do make a complaint to the Alberta Dental Association and College about the conduct of a dentist or a quality of care concern, your complaint must:

0 i' e5 `7 _/ ~* x8 Y/ ~1 b/ x jbe signed by you;

3 K+ g2 }4 l* m0 q3 N3 ^provide the name of the dentist you are complaining about;

include enough detail about the nature of your complaint so the dentist is able to respond to the complaint, such as what happened, when it happened and where it happened;

list your contact information including your mailing address and phone number; and
