POSITION SUMMARY

The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.* c* A- Q+ O7 U0 M" P4 W2 s

! {: e& `  W7 d, j2 GThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.; {3 I- A$ r& k. L  R
•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
- S+ X6 k" i5 C" F# J•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.
; d0 R1 O  M: i4 `% X•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.- l) ?3 X- O4 p7 G) w: v- U  S+ P
•        Educator, communicator, advocate, coordination/navigation, supporter# |  v5 X" g" ~& H( ]) i6 q

) E; ~- n8 w, {& ]2 B•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.

# `: @6 U  _" D9 u$ v! Y% Z7 k. D& c•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.
3 U( g0 {. N" E- q5 c•        Ensure the maintenance of accurate records in line with practice policy.
  Z$ y. z4 E0 T" S; v•        To assist in developing and implementing patient self-management programs.
# S! O6 Y5 Q7 u1 [•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.& N8 B' a& G% z6 F0 v, u9 X
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.

•        He/she should be able to work in a team while respecting the views/opinions of others.
& R' D( n, h" o, Q& z1 }•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.
  I& u6 G% z3 o1 l/ V7 q•        Requires the ability to work independently and prioritize workload.
; R& r+ j1 l$ P8 ~+ O& n) F" u1 N/ y6 ]•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.
% C# }0 w- u- X4 J1 S! @6 X0 B•        To participate in the Network’s education program.

) n7 a3 Y  m3 J- wSELECTION REQUIREMENTS:
# x  S1 }7 G& U9 U% w3 H2 a4 ~6 X  c- b- g% H: ^* q) ?
Qualifications/Experience& `7 a4 q8 S/ N
•        Baccalaureate degree in Nursing preferred- h4 ^) B; g3 F( h$ g
•        Preference given to those with at least 5 years nursing experience in primary care setting.
8 W3 ^; F. l8 \. V' j•        Current CARNA Registration
4 \- F2 `8 L1 }! W0 y, C2 [•        Current CPR – Healthcare Provider
0 {) X2 ^. O  S' h/ e•        Must have own vehicle and valid driver’s license  S/ _2 G2 e% _. _7 ~$ H
•        Knowledge of, and ability to use, electronic medical records, M# K6 t# {; `
