# a! d: s% n3 N- ]! P9 I3 YThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

+ q! ]& Y% `6 ? s) w2 q6 c) sThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

• To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. $ }6 d3 [$ a, C+ ?' c% n" m3 N2 d
• To develop long-term, trusting relationships with both patients and peers in providing primary care services.
( z N3 `6 K) E% s• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

• To assist in developing and implementing patient self-management programs.

SELECTION REQUIREMENTS:. x+ ~# M* B8 z9 F! f
+ S7 ]( P8 g f7 EQualifications/Experience
/ Z% s- w7 Y: w; K3 y4 V• Baccalaureate degree in Nursing preferred
7 D I- z, Z/ f8 p• Preference given to those with at least 5 years nursing experience in primary care setting.+ ^2 w1 T( a* r3 a
• Current CARNA Registration
$ p$ ]. Z1 m& M% Y• Current CPR – Healthcare Provider) q8 b7 p7 `1 u
• Must have own vehicle and valid driver’s license
