Primary purpose: The Social Worker works in partnership with settlement workers or
2 R: h* B1 ?4 `% T* s: Fcommunity cultural brokers from New Canadian Health Network partner organizations to j! p; e3 Q$ m2 ~
provide supplemental support to newcomers who have complex needs.

& z' C2 w! K2 k% T: M4 R% b Nature of Work: The Social Worker will report to the manager of mental health services. 4 I6 w( k( X. {# y2 G) Y8 E
The position builds working relationships with New Canadian Health Network partners’ staff,
) v% H! p8 s. x2 A; X t" n! j; ?% Gnewcomers accessing these services and other institutions and service providers. The Social % s- M1 g5 _, s! P1 R" z7 g4 G- m
Worker will assist in case planning, conferencing and providing support and advocacy as $ A4 W! u8 s# T; j' v
needed. As required, the Social Worker will assist in accessing community supports for 5 b1 l( j O) Q# S: X; c, T
newcomers and their families. The Social Worker provides holistic and flexible initial
3 ?# P+ G' [( I9 fassessment of life circumstances, needs, goals and aspirations and adheres to the client-5 E* `' q* _+ F; ?' V2 m9 K7 ?+ Y' Y
centered delivery model.

Providing holistic information or advocacy services for immigrants and refugees served. , T( `% T0 z( ?% u! J
Developing of social networks and community supports for newcomer families.

newcomers in settling and fully participating in community.

case plan support for clients who have complex needs.

Qualification

% V5 U& o0 Y1 t% z" s, W) D7 i J Relevant undergraduate degree or diploma, or equivalent combination of education and 1 S3 R+ d% o' Q! ^3 w. l1 n( B6 P0 j
experience in Social Work or related human service fields.
4 r! j- @, a; N% B" Q; C* o Extensive knowledge of community resources, including in depth understanding of family - Z9 s. U2 l$ J2 D+ s; g- s
support needs
# n9 A0 |- u8 Z Experience/facility with MS Office computer applications
5 d" f. L- N5 ?9 U5 c$ g Proficiency in both official languages is an asset, as is fluency in other languages
4 d& [ T8 v$ t Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
