! |, ?. P, F- H! u {( w! k3 } Nature of Work: The Social Worker will report to the manager of mental health services. / a* w/ W q3 d# M
The position builds working relationships with New Canadian Health Network partners’ staff,
+ ^9 Z; |) {( O/ R0 w$ v# ~) unewcomers accessing these services and other institutions and service providers. The Social ' L$ q5 P: N w9 L: |9 }
Worker will assist in case planning, conferencing and providing support and advocacy as
{* \% e. `( o7 U4 v$ sneeded. As required, the Social Worker will assist in accessing community supports for & m N5 z/ n9 E
newcomers and their families. The Social Worker provides holistic and flexible initial ; K7 ]' E+ _4 S6 b, n
assessment of life circumstances, needs, goals and aspirations and adheres to the client-3 V( G! z' F/ t, }& H
centered delivery model.

Understands and adheres to professional ethics related to social work.

Providing holistic information or advocacy services for immigrants and refugees served.

case plan support for clients who have complex needs.

Qualification 1 x6 c% S9 T2 Q3 D3 a3 F

Relevant undergraduate degree or diploma, or equivalent combination of education and
7 }+ P4 r; L2 _1 z, L$ K( Kexperience in Social Work or related human service fields.
! ]! k$ y: o% }2 o9 ^" b8 f Extensive knowledge of community resources, including in depth understanding of family
0 O; c2 L- r& F8 E v1 Lsupport needs
* w+ l, Y8 I3 @7 F( J* P: M Experience/facility with MS Office computer applications 7 } \) O1 P* F2 y! c
Proficiency in both official languages is an asset, as is fluency in other languages , v0 @% O% l6 J/ N# K
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 7 B( {5 I& I6 b1 m+ a2 x$ e( U
insurance coverage for work
