Primary purpose: The Social Worker works in partnership with settlement workers or

provide supplemental support to newcomers who have complex needs.

3 n8 f: p9 B1 w/ B Nature of Work: The Social Worker will report to the manager of mental health services.
3 n: u! ?/ g! `! j3 A" l$ pThe position builds working relationships with New Canadian Health Network partners’ staff,
1 e4 c# o" i- ]; F- o3 jnewcomers accessing these services and other institutions and service providers. The Social & D" S, K5 r- G9 h1 R
Worker will assist in case planning, conferencing and providing support and advocacy as 8 I. V7 I! B6 ?7 M) p
needed. As required, the Social Worker will assist in accessing community supports for ' m* j: e# N ]
newcomers and their families. The Social Worker provides holistic and flexible initial 0 s" r) n8 o' x% l
assessment of life circumstances, needs, goals and aspirations and adheres to the client-1 W$ J3 C" m+ N9 K# d; R% _4 D
centered delivery model.

Developing of social networks and community supports for newcomer families. 1 Y/ d1 V, L8 h
Provide intake, needs assessment, referral, case plans and action plans to support

@" b9 S# E0 rcase plan support for clients who have complex needs.

Qualification * k! I I2 `, [$ M; R
3 T6 u; i/ `5 O! w6 H: [5 A Relevant undergraduate degree or diploma, or equivalent combination of education and ' ^& c t1 y/ E$ s
experience in Social Work or related human service fields. @* i* G0 v2 ~ s5 l G5 }- m
Extensive knowledge of community resources, including in depth understanding of family 5 A( L1 f; R; ^/ W
support needs
( a* d# T7 x! {0 f4 E$ F Experience/facility with MS Office computer applications * H( L3 S* O- P8 F$ Q# I5 A7 a
Proficiency in both official languages is an asset, as is fluency in other languages 5 l6 H" j" W/ o, [& o; ?& J) {
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
