Primary purpose: The Social Worker works in partnership with settlement workers or 7 |# G+ f f' Q% l! ]& S" E
community cultural brokers from New Canadian Health Network partner organizations to
2 ?4 r- S( {" t) i3 dprovide supplemental support to newcomers who have complex needs.

+ A+ w8 E7 m( M/ ]0 F Nature of Work: The Social Worker will report to the manager of mental health services. B$ M; b8 ^. E
The position builds working relationships with New Canadian Health Network partners’ staff,
1 i3 o7 U/ B5 d6 d9 W6 ^newcomers accessing these services and other institutions and service providers. The Social
1 `& b# z. c$ G: @. y1 x* a: G. LWorker will assist in case planning, conferencing and providing support and advocacy as
! k0 n! l# S9 q ^4 ^needed. As required, the Social Worker will assist in accessing community supports for
' V. @* X3 h, Qnewcomers and their families. The Social Worker provides holistic and flexible initial / @5 P1 @3 ]. E) a( j" \
assessment of life circumstances, needs, goals and aspirations and adheres to the client-( G' Y" Y7 s! H6 f$ E
centered delivery model. $ X7 M) w9 Y' r. B/ m

Understands and adheres to professional ethics related to social work.

Developing of social networks and community supports for newcomer families. 7 w* G9 Y2 S! ? J
Provide intake, needs assessment, referral, case plans and action plans to support 3 f: v6 s# o0 }( B4 e2 d
newcomers in settling and fully participating in community.
% s; C5 T+ } G Work in partnership with settlement workers or cultural brokers to provide supplemental
, }+ ^& q* [( z, e/ Lcase plan support for clients who have complex needs.
! R" d7 m" m" J/ ]4 g Advocate on newcomers’ behalf to programs, institutions and service providers. ! W8 A' g. } v e3 B
Treat all clients with dignity and patience in order to listen deeply and give thoughtful

Qualification
6 w4 y: ]9 q( {% O 9 Z. Z9 B9 {7 ~
Relevant undergraduate degree or diploma, or equivalent combination of education and / l. j' t& ]1 L; ~+ J ]5 k4 r: M' `
experience in Social Work or related human service fields.
( s; S1 h# J1 r3 U7 e2 e Extensive knowledge of community resources, including in depth understanding of family
, k& A' K B& O5 }4 [support needs ; v" e7 v0 @1 X& L; }
Experience/facility with MS Office computer applications
; w' c" R9 ^! Q- z Proficiency in both official languages is an asset, as is fluency in other languages & {/ L8 R) D! a
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
7 u+ [( `+ d+ W8 v$ c* D3 kinsurance coverage for work

, B3 O8 p) A3 W9 NExperience: Minimum of 3 years experience working in a related field
