Abstract: position offers both crisis/on-going support for individuals/couples with chronic or complex mental health issues + those who face multiple barriers to independent living to enhance their ability to live successfully within their community by reducing barriers involved in meeting their economic, social and personal needs. 8 E- D0 _! s5 f: w+ |' ?1 b
0 ^$ m( n! ` `9 T6 v4 QPosition is also responsible for providing crisis intervention, intensive outreach support, advocacy, information and referrals to internal and external resources as appropriate. : b" _* t. c# [9 R, n9 b

E+ R1 |, t3 w7 JEducation/Experience: Bachelor Degree or Diploma in Social Work, Rehabilitation Services or equivalent + relevant experience; familiarity with issues of poverty/homelessness; AB Class 5 license, satisfactory drivers abstract and own vehicle.$ l; U% @1 W6 n* r4 u( e8 |8 P) W

Skills & Abilities: Strong written/verbal communication skills; effective assessment skills and ability to work from a strength-based perspective; ability to develop goal plans with the program participant; ability to establish rapport and trust with the program participants; strong problem solving skills and an ability to flexibly manage multiple tasks.$ q6 N" i5 \0 D6 _( s
) e# W$ O* o v. }. A# Y
PLUS: Knowledgeable about community resources; ability to be self-directed and work well within a team; ability to develop productive working relationships with program partners & other community agencies; ability to develop service plans with program participants and provide follow-up to support them in the achievement of their goals.% W, t2 t) H- K
