Target:        Low income families with children/youth aged 3-17 residing in Edmonton
8 y1 v/ q( h9 e! F) c" bDate        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
8 B* i+ Z1 B4 h( [  `- S2 CPlace        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
" P- S/ J; c) |+ k) n- y5 r  R+ T3. Each child is limited to 1 to 3 programs per organization
& K  O, L1 X5 b, L& K* F4. Information on Child’s swimming level when registering for swimming classes
5 ]7 O, G" b$ n  s5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

; E/ b% d, d" [& t! YWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
