Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
3 s! x* ~  J, x( T: aPlace        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
) U9 G1 P& [1 _, G/ t& dRemarks:
: r; a# ?: ]$ C1 C9 c1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
6 J& K9 q, B4 `% o' h% R2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
" z/ u$ d2 B: r2 k4. Information on Child’s swimming level when registering for swimming classes
' z9 X* P1 p' x" C5. Children are not required to be present at the Registration Day
; f3 |8 G7 y$ f# Z7 b/ x+ r6. Registrations will be accepted on a first-come, first-served basis

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
